صفحه 1429مقدمهدیابت یکی از شایع ترین بیماری ها در دنیا است که در بسیاری از موارد با عوارضی چون نفروپاتی همراه می باشد. همانند هر بیماری مزمنی دیابت می تواند با اختلالات خلقی از جمله افسردگی توام باشد. با این وجود بررسی ها در مورد فراوانی اختلال افسردگی اساسی و اختلال طیف دو قطبی در بیماران دچار نفروپاتی دیابتی محدود می باشد. در این مطالعه به بررسی فراوانی اختلال افسردگی اساسی و اختلال طیف دو قطبی در بیماران دچار نفروپاتی دیابتی و عوامل مرتبط با آن در شهر اصفهان پرداختیم.روش هااین مطالعه ی توصیفی مقطعی بر روی بیماران مبتلا به نفروپاتی دیابتی در سال 1390 در سطح شهر اصفهان صورت گرفته است. جهت بررسی افسردگی از پرسشنامه ی Beck و بررسی اختلال طیف دو قطبی از پرسشنامه ی MDQ (Mood disorder questionnaire) استفاده شد. اطلاعات به دست آمده با استفاده از نرم افزار SPSS نسخه ی 18 آنالیز گردید.یافته هادر این مطالعه تعداد 250 بیمار بررسی شدند که شامل 118 نفر (2/47 درصد) مرد و 132 نفر (8/52 درصد) زن بودند. فراوانی افسردگی در بیماران تحت مطالعه 8/84 درصد به دست آمد. همچنین 24 درصد بیماران مبتلا به اختلال طیف دو قطبی بودند. میانگین HbA1C (Hemoglobin A1C) در بیماران افسرده به طور معنی داری بالاتر از افراد غیر افسرده بود (01/0 = P).نتیجه گیرینتایج حاصل از این مطالعه نشان داد که فراوانی افسردگی در بیماران مبتلا به نفروپاتی دیابتی به طور چشمگیری بالاتر از جمعیت عمومی است و همین عامل می تواند اثرات نامطلوبی بر سیر پیگیری و درمان بیماران مبتلا به نفروپاتی دیابتی بگذارد.

هدف از این مطالعه، به دست آوردن نسبت میان غلظت فاکتور رشد مشتق از سلول های اندوتلیال عروق (VEGF یا Vascular endothelial growth factor) و گیرنده ی محلول آن (Soluble vascular endothelial growth factor receptor 1 یا SVEGFR1) در مایع زلالیه ی افراد مبتلا به رتینوپاتی دیابتی غیر پرولیفراتیو بود. همچنین ارتباط این نسبت با ادم ماکولا بررسی شد.
روش هااین مطالعه، یک مطالعه ی مقطعی تصادفی بود. مایع زلالیه حین تزریق آواستین در چشم افراد دچار رتینوپاتی دیابتی غیر پرولیفراتیو جمع آوری گردید و غلظت فاکتورهای VEGF و SVEGFR1 سنجیده شد. ضخامت متوسط ماکولا توسط دستگاه OCT (Optical coherence tomography) اندازه گیری شد.
یافته هانسبت VEGF به SVEGFR1 با ضخامت ماکولا ارتباط معنی داری داشت (46/0 = r؛ 05/0 = P) که بیانگر عدم تعادل بین VEGF و رسپتور محلول آن در مایعات چشم دچار ادم ماکولا بود.
نتیجه گیریکاهش غلظت SVEGFR1 باعث افزایش فعالیت VEGF می شود. از این طریق نفوذپذیری عروق شبکیه و ماکولا افزایش می یابد و این فاکتور پروآنژیوژنیک باعث افزایش ادم ماکولا می شود.

صفحه 1446مقدمهخواب ناکافی، کیفیت بد خواب و خواب آلودگی از مشکلات شایع مرتبط با یادگیری، حافظه و عملکرد تحصیلی در کودکان و نوجوانان است. با توجه به این که تاکنون در کشور ما مطالعه ای جهت بررسی ارتباط اختلالات خواب با عملکرد تحصیلی در نوجوانان مقطع راهنمایی انجام نشده است، جهت بررسی این ارتباط یک نمونه از دانش آموزان مقطع راهنمایی و والدین آن ها را در شهر اصفهان مورد مطالعه قرار دادیم.روش هامطالعه ی ما از نوع مقطعی بود. در این مطالعه 1100 دانش آموز مقطع راهنمایی شهر اصفهان با روش نمونه گیری تصادفی خوشه ایچند مرحله ای انتخاب شدند. اطلاعات از طریق پرسشنامه ی والد و خود گزارش دهی که روایی و پایایی آن در یک مطالعه ی آزمایشی ثابت شد (ضریب آلفای کرونباخ = 87/0). عملکرد تحصیلی از طریق نمرات ترم آخر تحصیلی دانش آموزان با استعلام از مدارس ارزیابی شد. نمرات بیشتر یا مساوی 17 به عنوان عملکرد تحصیلی عالی، نمرات 17-15 به عنوان عملکرد تحصیلی متوسط و نمرات کمتر از 15 به عنوان عملکرد تحصیلی ضعیف در نظر گرفته شد. برای تحلیل اطلاعات از نرم افزار SPSS نسخه ی 20 و برای تحلیل آماری از آزمون های 2χ، Student-t و One-way ANOVA استفاده گردید.
یافته ها565 نفر از افراد مورد بررسی دختر (4/51 درصد) و 535 نفر پسر (6/48 درصد) بودند. میانگین سنی آنان 09/1 ± 26/13 سال بود. 3/42 درصد افراد مورد پژوهش عملکرد تحصیلی عالی، 1/30 درصد عملکرد تحصیلی متوسط و 6/27 درصد عملکرد تحصیلی ضعیف داشتند. عملکرد تحصیلی به طور قابل توجهی با سن و جنس دانش آموزان، شغل و تحصیلات پدر و مادر آن ها ارتباط داشت. به علاوه بیدار شدن قبل از ساعت 6 صبح، دوره ی نهفتگی خواب بیشتر از 30 دقیقه و خواب منقطع شبانه باعث تضعیف عملکرد تحصیلی شد.
نتیجه گیرینتایج این مطالعه نشان داد که اختلالات خواب بر عملکرد تحصیلی نوجوانان به صورت منفی تاثیر می گذارد. با توجه به این یافته ها انجام اصلاحات و اتخاذ تدابیر مناسب در راستای کاهش پیامدهای سوء ناشی از اختلالات خواب ضروری به نظر می رسد.

صفحه 1458مقدمهپارکینسون یک بیماری استحاله ای سیستم عصبی مرکزی است که با علایم حرکتی و غیر حرکتی تظاهر می نماید. علایم غیر حرکتی شامل اختلالات اتونوم و اختلال شناختی و اختلال خواب است. بسیاری از علایم غیر حرکتی از جمله اختلال خواب شایع می باشد و به طور بارز در فعالیت های روزانه ی بیمار و کیفیت زندگی بیماران تاثیر می گذارد. مهم ترین هدف مطالعه ی حاضر، بررسی کیفیت خواب بیماران مبتلا به پارکینسون بود.روش هااین مطالعه ی مقطعی در سال 1390 روی بیماران مبتلا به پارکینسون که به درمانگاه اعصاب بیمارستان الزهرای (س) اصفهان مراجعه کردند، انجام شد. 34 بیمار مبتلا به پارکینسون و 34 نفر از افراد سالم به عنوان گروه شاهد در این مطالعه شرکت کردند. کیفیت خواب بیماران با پرسشنامه ی PDSS (Parkinson''s disease sleep scale) ارزیابی شد. میانگین نمره ی PDSS در دو گروه مورد مطالعه توسط آزمون Student-t مقایسه گردید.یافته هامیانگین نمره ی PDSS در بیماران مبتلا به پارکینسون 92/26 ± 99/55 بود که نشان دهنده ی اختلال خواب متوسط تا شدید بود. میانگین نمره ی PDSS در گروه شاهد 65/10 ± 34/20 بود. تفاوت نمرات دو گروه معنی دار بود (05/0 > P).نتیجه گیریمطالعه ی ما نشان داد که کیفیت خواب در بیماران مبتلا به پارکینسون نسبت به گروه شاهد پایین تر است.

صفحه 1464مقدمهسپتی سمی، سندرمی به سرعت پیشرونده و تهدید کننده ی حیات است که به صورت سندرم پاسخ التهابی سیستمیک ناشی از عفونت تعریف می شود. تشخیص صحیح و سریع این سندرم جهت حصول نتایج بالینی قابل قبول و کاهش مرگ و میر ناشی از آن بسیار مهم است. هدف از این مطالعه، بررسی اعتبار کشت خون که یکی از شایع ترین آزمایش های مورد استفاده در تشخیص سپسیس می باشد، نسبت به علایم بالینی سندرم پاسخ التهابی سیستمیک (Systemic inflammatory response syndrome یا SIRS) ناشی از سپتی سمی در بیمارستان های آموزشی شهر اراک بود.روش هادر این مطالعه مشاهده ای تحلیلی، 289 بیمار بستری در بیمارستان های آموزشی شهر اراک که برای آن ها کشت خون انجام شده بود، بر اساس معیارهای ورود (رضایت بیمار یا ولی وی، عدم گذشت بیش از 24 ساعت از شروع آنتی بیوتیک تزریقی و عدم گذشت بیش از 24 ساعت از زمان انجام کشت خون) وارد مطالعه شدند. سپس نتایج کشت خون در دو گروه SIRS+ و SIRS- ارزیابی و حساسیت و ویژگی آن تعیین شد.
یافته هادر نمونه های مورد بررسی 215 نفر (6/74 درصد) SIRS+ و 74 نفر (6/25 درصد) SIRS- بودند. از میان نمونه های کشت شده نیز 29 بیمار (03/10 درصد) کشت خون مثبت داشتند که 9/6 درصد آن ها با علایم بالینی SIRS+ مطابقت داشت و 1/3 درصد آلودگی بود. حساسیت و ویژگی کشت خون به ترتیب 3/9 و 84/87 درصد محاسبه شد.نتیجه گیریبا توجه به اهمیت تشخیص صحیح و سریع سپتی سمی و حساسیت پایین کشت خون به نظر می رسد توجه به علایم بالینی بیمار و انجام آزمایش با توجه به اندیکاسیون ها و استانداردهای آن از اهمیت بالایی برخوردار است.

Page 1429BackgroundDiabetes is the most prevalent disease in the world. It is in many cases associated with complications such as nephropathy. Like other chronic diseases، diabetes can be accompanied by mood disorders including depression. However، studies about the frequency of major depressive disorder and bipolar disorder among diabetic patients are limited. In this research، we surveyed the frequency of major depressive disorder and bipolar disorder in patients with diabetic nephropathy.MethodsThis cross-sectional research was performed on 250 patients with diabetic nephropathy in Isfahan، Iran during 2011. Beck Depression Inventory and Mood Disorder Questionnaire were used to assess depression and bipolar disorder، respectively. Data was analyzed with SPSS18.FindingsMales and females constituted 47. 2% (n = 118) and 52. 8% (n = 132) of the studied population، respectively. The frequency of depression was 84. 8%. Moreover، 24% of the patients had bipolar disorder. The mean of hemoglobin A1C (HbA1C) level of depressed patients was significantly higher than non-depressed patients (P = 0. 01).ConclusionThe prevalence of major depression is significantly higher in patients with diabetic nephropathy compared to the general population. The goal of this study was to determine the ratio of vascular endothelial growth factor (VEGF) to soluble VEGF receptor 1 (SVEGFR1) in aqueous humor of patients with non-proliferative diabetic retinopathy (NPDR). We also investigated the association between this ratio and macular edema.
MethodsIn this randomized، cross-sectional study، aqueous humor was collected during Avastin injection in ocular fluid of patients with NPDR. Concentrations of VEGF and SVEGFR1 were then determined. Macular thickness was measured with ocular coherence tomography (OCT).
FindingsVEGF/SVEGFR1 had a significant correlation with macular thickness (r = 0. 46; P = 0. 05). Therefore، imbalance between VEGF and its soluble receptor in ocular fluid is related with macular edema.
ConclusionDecreased level of SVEGFR1 increases the activity of VEGF. As a result، vessels in retina become permeable. This pro-angiogenic factor will in turn increase macular edema.

Page 1446BackgroundInsufficient sleep، poor sleep quality، and sleepiness are common problems in children and adolescents. Such problems can affect learning، memory، and academic performance. No previous studies have assessed the relationship between sleep problems/habits and academic performance in Iranian secondary school students. Therefore، we surveyed a sample of secondary school students and their parents in Isfahan (Iran) to assess the relation between sleep problems/habits and academic performance.MethodsIn this cross-sectional study، data was collected by a reliable (Cronbach’s alpha = 0. 87) and valid self-reporting questionnaire. Multi-stage cluster random sampling was used to select 110 secondary school students in Isfahan، Iran. Academic performance was assessed using the latest term evaluation report provided by the school. Students’ performance was stratified as excellent [grade point average (GPA) ≥ 17]، average (GPA = 15-17)، and poor (GPA < 15). The collected data was analyzed with SPSS20. Descriptive statistics (mean ± standard deviation)، independent t-test، chi-square test، and analysis of variance (ANOVA) were used to analyze the data.FindingsThere were 535 boys (48. 6%) and 565 girls (51. 4%) in this study. The mean age of students was 13. 26 ± 1. 09 years (range: 11-16 years). While 42. 3% of the students had excellent performance، 30. 1% and 27. 6% of them had average and poor performance، respectively.ConclusionAcademic performance of the students was significantly associated with their age، gender، and fathers and mothers’ education levels and jobs. It was evident that the presence of sleep problems، poor sleep habits، or shortened total sleep time affected the students’ academic performance. Page 1458BackgroundParkinson’s disease (PD) is a progressive neurodegenerative disorder causing motor and non-motor symptoms. The latters are common and include autonomic dysfunction، cognitive impairment، and sleep difficulties. Many of the non-motor aspects of PD such as sleep disturbance are more common and significantly affect everyday activities and quality of life of patients. The most important aim of this study was to evaluate sleep quality in patients with PD.MethodsIn this case-control study، 34 patients with PD and 34 healthy controls were included. The patients had referred to the Neurology Clinic of our teaching hospital (Isfahan، Iran) in 2011. Sleep quality of all subjects was evaluated by Parkinson’s Disease Sleep Scale (PDSS) which is a reliable and valid tool to measure sleep disorders in PD.FindingsThe mean total PDSS score in patients was 55. 29 ± 26. 92 which indicated moderate to severe sleep disturbances. The mean total score in the control group was 20. 34 ± 10. 65. Page 1464BackgroundSepsis is a rapidly progressive and life-threatening syndrome. It is defined as a systemic inflammatory response syndrome (SIRS). In order to achieve acceptable clinical outcomes and reduce mortality and morbidity associated with sepsis، accurate and timely diagnosis of this syndrome is very important. The aim of this study was to evaluate the validity of one of the most common tests used in the diagnosis of sepsis (blood culture) with SIRS in teaching hospitals in Arak، Iran.MethodsIn this analytical study، 289 patients who had been admitted to teaching hospitals in Arak and consented to participate were evaluated. Patients were only included if they had received intravenous antibiotics for less than 24 hours and their blood culture was performed during the past 24 hours. The results of blood cultures were evaluated in two groups of SIRS+ and SIRS-. The sensitivity and specificity of the test were then determined.FindingsOf 289 studied samples، 215 (74. 6%) patients were SIRS+ and 74 (25. 6%) were SIRS-. In cultured samples، 29 (10. 03%) were positive but only 6. 9% were true positive. However، 3. 1% of the samples were contaminated. The sensitivity and specificity of blood culture were 9. 3% and 87. 84%، respectively.ConclusionAccording to the importance of accurate and timely diagnosis of sepsis and low sensitivity of blood culture، it seems that decision making based on clinical symptoms and testing indications and standards is essential.Keywords: Blood culture validity, Blood culture, Sepsis, Systemic inflammatory response syndrome
